Application Information

If you are interested in joining Health Temp's team of healthcare professionals, you can either fill out our Online Application or come and visit us at one of our convenient office locations in Mesa, Phoenix, or Tucson.

Please bring the following items with you when applying at Health Temp:

		Nursing License (if applicable to position)
		Current CPR certification card
		Current advanced certification cards (i.e. ACLS, PALS, NRP, etc.)
		Information about education, previous employers, and references
		Immunization records (Rubella, Rubeola, Varicella, etc.), if available.
		TB test results, if available.
		Voided check (if you wish to be paid on direct deposit payroll)
		Two valid forms of identification or your passport

During the application process, we will have you:

		Fill out an application, including work and educational history information
		Complete tax forms
		Take competency test(s) for area(s) you wish to work
		Fill out competency checklists including years worked, for the area(s) in which you intend to apply
		Read and watch in-service materials and videos
		Please allow yourself 1 to 1 ½ hours to complete the application process
